Enrollments
RE-ENROLLMENT: All 4-H re-enrollment forms are due to the UW-Extension Office by December 1, 2011. NO forms will be accepted without a leader signature. Check with the club leader to see if enrollment forms should be returned to the leader or returned to the Extension Office.
NEW ENROLLMENT: NEW 4-H Member enrollments are accepted throughout the year, BUT if you would like to show at the 2012 Iowa County Fair your enrollment needs to be turned into the your Club Leaders by APRIL 1st.
Remember that youth in 3rd grade this year and were in Cloverbud 3 last year have become full 4-H members and are able to choose from any of the projects listed on the attached sheet. Youth in K-2nd grade can choose from Cloverbud 1, Cloverbud 2 or Cloverbud 3.
There is an enrollment fee of $5 per youth member or $15 for a family of three or more. The enrollment fee must be submitted with the enrollment form. Please make checks payable to: Iowa County 4-H.
Leader’s re-enrollment forms include Behavior Expectations on the form so leaders only need to sign one sheet. Leaders will need to have the club leader sign the form. Check with the club leader to see if enrollment forms should be returned to the leader or returned to the Extension Office.
